Vancomycin and Teicoplanin in Adults CA2030 v7
The aim of this guideline is to offer guidance on vancomycin and teicoplanin dosing and monitoring for adult patients who receive more than one dose excluding the Critical Care Complex where vancomycin is given by continuous infusion and in theatres where teicoplanin is used for surgical prophylaxis .
